Declared his intention to become a citizen of the US on April 15, 1904 in Common Pleas Court of Cuyahoga County.
Wife: Annie O'Rourke, who was born in Cleveland and resides there.
Children: Ellen, born November 18, 1905; Mary, born March 25, 1907; William, born October 5, 1908. All born in Cleveland and residing in Cleveland.
Renounces allegiance to Edward VII, King of Great Britain and Ireland.
Can speak the English language.
Has resided in the United States since June 27, 1899, and in Ohio since June 28, 1899.
